It’s very straightforward: your full name, your mailing address, your social security number, your annual income, and the name of your employer. Even if you’re applying for a prepaid card, not a credit card, you’ll need to fill out this information, except maybe for income. Keep in mind that you do not have to show tax returns, provide a letter from your employer, or furnish any other proof of income or employment. Just fill it out to the best of your ability and answer honestly. It’s important that credit card issuers have all of this information, and that it is accurate, so they can assess your creditworthiness and open up a line of credit for you that is appropriate to your means to pay back the debt. They will run a credit check on you once you apply, and you will usually have your answer within minutes, if you apply online.
